D. I. Pisarev belongs to the brilliant galaxy of Russian materialist thinkers, along with Belinskii, Herzen, Chernyshevskii, and Dobroliubov. He was a passionate, fighting essayist and an original thinker who played a role of the very highest order in providing a rationale for, and upholding, materialism and revolutionary ideas for the transformation of society. Pisarev ruled the minds of the young generation of his day: he was their recognized authority and preceptor in the realm of ideas. An approach to constructing counterparts of some fields of mathematical analysis in the frames of Pilbert's “finitary standpoint” is sketched in this paper. This approach is based on certain results of functional spaces theory development in classical mathematics.

Stiff Person Syndrome is a rare autoimmune disorder associated with antibodies against glutamic acid decarboxylase, the key enzyme in γ -aminobutyric acid synthesis. In order to investigate the role of cerebral benzodiazepinereceptor binding in SPS, we performed [ 11 C]flumazenil positron emission tomography in a female patient with SPS compared to nine healthy controls. FMZ is a radioligand to the postsynaptic central benzodiazepine receptor which is co-localized with the GABA-A receptor. In the SPS patient, we found a global reduction of (...) cortical FMZ binding. In addition, distinct local clusters of reduced radiotracer binding were observed. These data provide first in vivo evidence for a reduced postsynaptic GABA-A receptor availability which may reflect the loss of GABAergic neuronal inhibition in SPS. (shrink)

Background: Discussing treatment risks has become increasingly important in medical communication. Still, despite regulations, physicians must decide how much and what kind of information to present. Objective: To investigate patients’ preference for information about a small risk of a complication of colonoscopy, and whether medical and personal factors contribute to such preference. To propose a disclosure policy related to our results. Design: Vignettes study. Setting: Department of Gastroenterology, Academic Medical Centre, the Netherlands. Patients: 810 consecutive colonoscopy patients. Intervention: A home-sent (...) questionnaire containing three vignettes. Vignettes varied in the indication for colonoscopy, complication severity and level of risk. Patients were invited to indicate their wish to be informed and the importance of such information. In addition, sociodemograhic, illness-related and psychological characteristics were assessed. Main outcome measurements: Wish to be informed and importance of information. Results: Of 810 questionnaires, 68% were returned. Patients generally wished to be informed about low-risk complications, regardless of the indication for colonoscopy or the severity of the complication. The level of risk did matter, though (OR = 2.48, SE = 0.28, p = 0.001). The information was considered less important if done for population screening purposes or diagnosis of colon cancer, if the complication was less severe (bleeding) and if the risk was smaller (0.01% and 0.1%). Patients’ information preference was also related to age, mood and coping style. Limitations: Difficulty of vignettes. Conclusions: Patients generally wish to be informed about all possible risks. However, this might become uninformative. A stepwise approach is suggested. (shrink)

Correct interpretation of the data from integral laboratory tests, including Thrombin Generation Test, requires biochemistry-based mathematical models of blood coagulation. The purpose of this study is to describe the experimental TGT data from healthy donors and hemophilia A and B patients. We derive a simplified ODE model and apply it to analyze the TGT data from healthy donors and HA/HB patients with in vitro added tissue factor pathway inhibitor antibody. This model allows the characterization of hemophilia patients in the space (...) of three most important model parameters. The proposed approach may provide a new quantitative tool for the analysis of experimental TGT. Also, it gives a reduced model of coagulation verified against clinical data to be used in future theoretical large-scale modeling of thrombosis in flow. (shrink)

A. N. Williams examines the conception of the intellect in patristic theology from its beginnings in the work of the Apostolic Fathers to Augustine and Cassian in the early fifth century. The patristic notion of intellect emerges from its systematic relations to other components of theology: the relation of human mind to the body and the will; the relation of the human to the divine intellect; of human reason to divine revelation and secular philosophy; and from the use of the (...) intellect in both theological reflection and spiritual contemplation. The patristic conception of that intellect is therefore important for the way it signals the character of early Christian theology as both systematic and contemplative and as such, distinctive in its approach from secular philosophies of its time and modern Christian theology. We can best begin from Wilson's "simple little puzzle" about Caesar and Antony: "What would the world be like if Julius Caesar had all the properties of Mark Antony and Mark Antony had all the properties of Julius Caesar?" Wilson's own approach to an answer is indirect--he begins by telling us not what such a world would be like but what it would look like. "Clearly the world would look exactly the same under our supposition." But this assumes that the (...) question "What would such a world look like?" is a proper one; which it surely is not. For his answer to it is meaningless until he specifies to whom this supposed world would look as he says it would. It would look exactly the same to him or to me; but would it have looked the same to Caesar or to Antony? In fact Julius Caesar had the experiences of being called "Julius Caesar," being murdered on the Ides of March, and so on, and these are very different experiences from being called "Mark Antony," dallying on the Nile with Cleopatra, and so on; so I don't see how this alternative course of events could possibly have looked the same to Julius Caesar; or--using a similar line of argument--to Mark Antony. So I cannot agree that, as Wilson goes on to say, "our attempt to describe a distinct possible world has produced just the same old world all over again." I am not, indeed, convinced that even a world which looked to everyone just as the actual one does would necessarily be the same world ; but even putting this doubt aside, since the world mentioned wouldn't look to everyone as the actual world does, it wouldn't be the same even by Wilson's own standards.
